Employee Dies in Fall from Roof

At approximately 4:00 p.m. on January 16, 2008, Employee #1, a roofer, was reroofing a four-story apartment building. The top of the flat roof was 30-ft by 11-ft, surrounded by a guard rail, except in limited locations. The project was expected to last four days and involved a general cleaning of the roof with brooms, followed by application of a thin roll-on membrane using torches and then the application of metallic edge flashing. No personal fall protection was present or used on the job site because the owner did not believe that fall protection was required for roofing work on flat roofs. Employee #1, while working on a drain hole at the edge of the roof, stepped backwards and fell 38 feet over the roof's edge to his death. The foreman was present on top of the roof and within visual range of the worker at the time of the fall.
