Investigation Summary
At approximately 1:30 p.m. on June 30, 2007, Employee #1 was stabilizing a window sash that had been inserted from the inside of a building. He was on the eighth rung of a 24 ft Werner Type 1A orange fiberglass extension ladder when he lost his balance. He fell approximately 8 ft 8 in. to the right of the ladder, landing on the tiled front entrance. Employee #1 sustained a fractured right wrist, a concussion, and lacerations to the left side of his forehead and to his left lower leg. He was transported to Marin General Hospital, where he was treated and released on July 2, 2007. Subsequent investigation found that the accident was caused by failure to secure the portable ladder.
