Employee Amputates Fingers on Table Saw

At approximately 2:00 p.m. on June 1, 2007, Employee #1, a finish carpenter, was building shelves and trim for a residential remodeling project. He was using a Bosch 4000 table saw and rip-cutting 1-inch by 4-inch wood with a 10- inch blade. The saw was equipped with a blade guard, spreader, and anti-kickback device. Employee #1 was cutting 1-inch widths from a 4-ft piece of wood to construct a baseboard. He did not adjust the height of the blade to minimize blade exposure, leaving 1-inch to 1.5-inch of the blade projected above the workpiece. Employee #1 was pushing the work piece with both hands into the blade, using a plastic push stick in his right hand, and pushing with his hands 4-inch to 6-inch away from the blade at the edge of the table saw top. As he worked, a coworker walked into the room and distracted him. His left hand moved and contacted the front of the blade guard. The blade guard moved upward, and his left hand contacted the blade, severing his ring and middle finger. Employee #1 and a coworker walked to a nearby hospital, where he was admitted to have his fingers reattached.
