Investigation Summary
At approximately 12:45 p.m. on October 24, 2006, an employee was receiving a stack of wooden corbels, each corbel was approximately 12-in. by 12-in. by 6-ft, and up weighed to 300 pounds each, arranged in three rows of three corbels per row. The stacks of corbels were banded with two metallic bands perpendicular to the longest dimension of the corbels. The stack had just been delivered by a front end loader equipped with forks, and the stack was on top of the forks at the time of the accident. The employee, standing in front of the longest dimension of the corbels, cut both metallic bands and the row of corbels closest to him. The corbel fell over on his leg, breaking both lower leg bones in right leg. Employee was taken to St. Luke's Hospital in San Francisco, where he was hospitalized and surgery was performed on his leg.
