Worker's Foot Is Run over by Forklift Truck

At approximately 8:00 a.m. on October 27, 2006, a construction foreman was using a forklift truck to unload sheetrock from a truck. Three other employees were helping at the site. As the forklift operator was backing up, he ran over an employee's foot. After he ran over his foot, the foreman stopped and helped the injured employee. The emergency medical technician on site started to treat the injured employee. An ambulance took the employee to San Francisco General Hospital for treatment and hospitalization. In his statement to accident investigators, the foreman stated that he looked over his shoulder and saw no one in his path. He stated that he did not see the employee who was approaching his forklift to help unload sheetrock. The OSHA investigation concluded that the forklift operator did not look in the direction of travel and did not operate the forklift without being certain that his path was clear. In addition, the forklift did not have a backup alarm.
