Employee Injures Head in Fall

At approximately 8:30 a.m. on June 13, 2006, Employee #1, a warehouseman, and a coworker were working for a building supply company that was a wholesaler of floor coverings, cabinets, and stone countertops. While the warehouse manager was in his office, Employee #1 and his coworker were filling an order for bamboo flooring for a customer. The bamboo flooring was stored in boxes measuring about 8 inches by 8 inches by 36 inches, and weighed 60 pounds. These boxes each had two straps, tied on either end, and were located on a pile of merchandise that was 8 feet above the ground. Employee #1 and his coworker climbed several boxes of merchandise to reach the top of the pile and began loading a forklift that contained an elevated pallet. As Employee #1 handled the boxes by their straps, tossing them onto the pallet, a strap broke. Employee #1 lost his balance and initially fell onto a 5-foot pile of merchandise and then landed on a concrete floor, striking his head. Emergency services were contacted and Employee #1 was taken to San Francisco General Hospital, where he died.
