Employee Suffers Lacerations in Forklift Accident

Employee #1 is a Plant Operator at a rendering plant. At the time of the accident, Employee #1 was driving a forklift (Hyster, Model Number H50XM, Serial Number D177B34540T, S10251766) inside the plant processing area. The forklift was carrying a press, weighing approximately 500 pounds, used in the press machine. This is below the maximum capacity of the forklift (5100 lbs). The forklift is 43-in. wide and 12-ft long. The aisle where the forklift was traveling on was 48-in. wide at the narrowest point. The narrow aisle had an elevated area, approximately 3.5-in. higher than the permanent floor. Employee #1 was holding the left canopy upright with his left hand as he backed up to get around a tight and sharp corner. The right rear tire (solid tire) hit the elevated surface of the floor and the forklift slid to the left side, pinching Employee #1's upper left arm between the rear left canopy upright and a steam valve. Employee #1 sustained a 5-centimeter deep laceration on his upper left arm and was hospitalized for two days.
