Investigation Summary

Investigation Nr: 307391672
Event: 02/15/2005
Employee Is Struck by Column

At approximately 12:30 p.m. on February 15, 2005, Employee #1, a laborer, was working for Michael Heavey Construction, a subcontractor doing concrete work, remodeling a residential house. The crew was in the process of removing an I-beam column approximately 20 feet in length through a floor opening, with a "Gradali" forklift. The column was hoisted up past the third floor and its bottom plate snagged at the opening. Employee #1 tried to free the jam. When the column was freed it pivoted and the heavier upper end came down, striking Employee #1 on the right forearm as he retreated. Employee #1 was taken to SF General Hospital by ambulance, where he was treated for multiple fractures to his right forearm. He was hospitalized for three days.
