Employee Amputates Finger in Table Saw

On May 26, 2004, Employee #1, an assembler, was producing architectural woodwork in San Francisco, California. Employee #1 was operating a "Casadei" KS-15 table saw. He was cutting a piece that was approximately 8 feet long by 2 feet wide by 0.75 inches thick for cabinet trim and footings. He had a push stick in his right hand. As he came to the last inch of the piece, it started to vibrate and rattle. At approximately 11:20 a.m., the piece he was cutting bounced away and his right hand went towards the rotating saw blade. The employer took Employee #1 to San Francisco General Hospital. Employee #1 had amputated his right index finger to the first digit, and the hospital sewed together the first digit of his right middle finger, and stitched his lacerated right fifth little finger.
