Investigation Summary
On February 12, 2004, Employee #1 was operating a scissor lift to change an overhead light. The lift was fully extended when it's right front wheel rolled off the asphalt roadway edge, causing it to turn over. Employee #1 fell approximately 30 feet and was killed.
