Employee Is Killed When He Fell from Concrete Bridge

On April 12, 2004, Employee #1 walked across the top of a concrete bridge beam to the area he was to work at. Upon arriving at the work location, Employee #1 apparently either jumped or fell approximately 4-ft from the top of the concrete bridge beam to the decking, which was supported by lagging between the lower flanges of two bridge beams. The 4-in. by 4-in. lagging and the 0.5 in. plywood sheet upon which Employee #1 landed broke, allowing him to fall 30 ft to the ground below. Employee #1 was killed.
