Investigation Summary
On July 3, 2005, Employee #1 was supervising the operation of a paper making machine. It was shutting down due to a break in the paper. There was a rope break as well, but a rope break, unlike a paper break, does not require that the machine stop. The machine, which ordinarily has paper running through it at a speed of 2,400 feet per minute, was coasting to a stop. Employee #1 was apparently checking the area of the paper break as the machine was slowing down. There were no witnesses to the accident, but evidence indicates that he was somehow caught by the large rollers and broken ropes of the paper machine. The ropes may have pulled him into the machine and up over one of the large rollers. Employee #1 then fell approximately 18 to 20 feet to the concrete floor in the basement below the paper machine. Employee #1 was killed. He was found dead at the scene.
