Employee Is Crushed and Killed by Fork Lift Mast

Employee #1, the operator of a forklift, was an untrained, unauthorized operator. Employee #1 was instructed by a supervisor, who knew that the employee was untrained, to park a forklift. After telling Employee #1 to park the forklift, the supervisor went back into the building, leaving Employee #1 unsupervised. According to witnesses, Employee #1 started playing around on the forklift, making it do donuts on the asphalt parking area. According to the witnesses other coworkers were encouraging Employee #1 and telling them to watch him. Employee #1 ran the mast all the way up, looked at the other coworkers and motioned for them to watch again. Employee #1 accelerated rapidly, turned the steering wheel sharply, causing the forklift to turn over. Employee #1 was not wearing the seat belt that was installed on the forklift. Employee #1 either jumped from the forklift or was thrown from it as it began to roll over. Employee #1 landed on his feet and according to the witnesses was trying to run from the overturning lift when the mast caught him, crushing him down into a sitting position under the mast. Employee #1 was killed.
