Investigation Summary
On May 19, 2004, Employee #1 was using a tester intended for shorting an electrical motor to determine if there was a problem with the motor's rewind component. The tester was designed to step the voltage up from 120 volts to 1,100 volts. The tester consisted of two self-retracting probes, one to be placed on the frame on the electrical motor, while the other in contact with the machines wires. However, one of the self-retracting probes was damaged, containing two base spots which exposed its conductor, and would not retract. As Employee #1 held both probes in his hands, contacting the machines frame and wires, he accidentally touched the energized portion with his hands. This caused 1,100 volts to go through his body, killing him.
