Investigation Summary
On January 3, 2006, Employee #1, a building surface applicator with J & J Acoustics, Inc., was applying a surface coating to a building. He was on a metal scaffold that had been erected on the roof of an adjacent building that was three stories shorter. The two buildings were separated by a distance of 22 in. Between the buildings, a foreman had erected a platform of 0.5-in.-thick plywood to catch debris. The platform sections were 8-ft long, 24-in. wide, and connected with nails. It was raining on the day of the accident. Employee #1 was reportedly wearing cotton gloves with rubber palms. When he climbed outside the scaffold bracing and stepped onto the debris platform, two of its sections separated. Employee #1 then fell over 51 ft to the ground between the buildings. He died from his injuries on January 10, 2006.
