Investigation Summary
At approximately 11:20 a.m. on July 25, 2005, Employee #1 was assisting a coworker to perform maintenance on a piece of equipment. The coworker was located on a platform while Employee #1 was on the ground below him, providing the coworker with parts and tools. A molding machine was located underneath the platform that the coworker was standing on. While Employee #1 was handing an air hose to his coworker, he stepped onto the molding machine. He then stepped down from the machine onto a Draw Frame bar, twisted his ankle, and fell approximately 1.5 feet onto the concrete floor. Although Employee #1 was experiencing discomfort, he did not seek medical treatment immediately. Five days after the accident, Employee #1 sought medical attention and was diagnosed with a left hip fracture. Employee #1 was hospitalized at Summit Medical Center and underwent surgery to have a pin inserted into his fractured hip.
