Employee's Fingers Are Amputated in Bending Machine

On May 3, 2005, an employee was using a Niagara brand, Model Number G-72, bending machine. His task was to roll 0.375-in. brass bar stock into coils. Depressing or releasing a foot pedal started and stopped the rollers. A metal shroud covered the pedal and prevented inadvertent activation of the rollers. While rolling the second bar into a coil, the stock started to fall over, which would have produced a twist in the coil. The employee grasped the stock with his right hand and attempted to take his foot off the pedal. His work boot was stuck in the shroud. He couldn't get his foot off the pedal or out of the shroud. The rollers continued to turn and pulled the third and fourth fingers of his right hand between the rollers and the stock. The squeezing action of the rollers amputated the middle and distal phalanges of his fourth finger and lacerated his ring finger. The employee was not hospitalized.
