Employee's Hand Is Crushed Between Forklift and Storage Rack

On February 28, 2005, an employee was assigned to operate a forklift in the shipping and receiving area at the Secondary Employer. The employee was operating the forklift in reverse gear, when he wrapped his right hand around the right-rear upright of the forklift's Rollover Protection System (RPS). While his hand was in that position, the employee backed toward a storage rack holding inventory to be shipped. The employee was reportedly traveling at a slow speed. When he applied the brake, one of the rear wheels of the forklift struck a piece of pallet that was on the floor, and the forklift skidded into the storage rack. The right-rear upright of the forklift's RPS collided with the frame of the storage rack at the point where the employee's hand was holding the upright. The employee's hand was crushed, and the palm degloved, between the metal frame of the rack and the right-rear upright of the forklift's RPS. The hand was partially amputated. The employee's injuries resulted from the shearing and crushing action of the frame of the forklift's RPS striking the frame of the storage rack. He was hospitalized.
