Employee Is Killed after Drving Off Dock

At approximately 3:22 p. m. on November 9, 2004, Employee #1, a warehouseman, was working for his employer, a steel products manufacturer. Employee #1 was operating a stand-up industrial truck to move product to the stacking area when he noticed another truck in his staging area. Employee #1 left his vehicle and entered the other vehicle to move it out of his way. Employee #1 placed the truck in reverse and accidentally drove off the edge of the dock and was killed. Other employees in the area had tried to warn him but Employee #1 was unable to stop in time. At approximately 3:40 p. m. the employer notified the Oakland District Office of the California Department of Occupational Safety and Health. On November 23, 2004, the truck manufacturing company conducted a reenactment and determined the braking system was working properly.
