Employee amputates finger with eye forming machine

On April 23, 2004, an employee, worked as a shop maintenance mechanic was called to work on the eye forming machine that had not been used for many months prior to the incident. The production crew had attempted to use the machine but was unable to get the machine to turn on. The employee worked on the machine for a couple hours attempting to restore service. While the employee was performing maintenance, the control circuit was inadvertency completed causing the machine to cycle, crushing the employee's left hand. The employee was hospitalized and treated for amputation of three fingers on his left hand.
