Employee Amputates Fingers in Table Saw

On June 28, 2004, an employee was assigned to his usual and customary occupation as a production worker. He was asked by his supervisor to cut pieces of aluminum stock on a table saw. The employee had used the saw several times a day for the past 5 months. The saw was a Powermatic brand electric table saw, Model Number 72A, Serial Number 0104072071. The material was aluminum, basically "L" shaped, about 9 inches long and 0.125 inches thick. The height of the saw blade was set at 0.156 inches. The saw was appropriately guarded. The supervisor ran the first five pieces of the stock to show the employee how to make the cut. He pushed the material past the blade by hand. The supervisor observed that the employee could make the cut following his directions, and then walked away. As the employee cut the next piece, the stock twisted against the blade and kicked back. His fingers were cut. He was hospitalized and treated for amputation.
