Investigation Summary
At approximately 4:30 p.m. on July 29, 2009, Employee #1 was installing a glass roof on an atrium. He fell through an open hole approximately 20 ft to the concrete floor. He was transported to a local trauma center, where he died at 8:00 p.m. that evening. Employee #1 was wearing a personal fall arrest harness; however, he was not tied off.
