Employee falls from wire rope ladder and is killed

At approximately 8:25 p.m. on May 6, 2004, an employee climbed a wire rope ladder to access a suspended lighting truss above a concert stage. As he approached the top of the ladder, he lost his balance and was killed when he fell approximately 30 feet to the stage. He was not wearing a fall protection device.
