Employee Dies after Fall from Elevated Work Platform

Three employees were preparing a toolbox truck to be shipped over the road. The crew was securing an air conditioning unit onto the trailer, which was located approximately 2 meters above the ground. One of the employees was working from an unsecured personnel platform, which was on a pallet supported by an industrial truck. When that employee leaned over to lower a frame, the platform tipped over and fell off of the pallet. He sustained severe head injuries and died later at a hospital.
