Employee's arm amputated by steel beams

At approximately 3:15 a.m. on May 28, 2004, Employee #1 was hoisting a bundle, which consisted of four I- Beams, from a steel beam storage yard. A lifting device, known as a "beam-puller," slipped out of the webbing of the bottom beam. Employee #1's right arm was trapped underneath the four beams and amputated, just below the elbow. Employee #1 was hospitalized.
