Investigation Summary
At approximately 3:00 p.m. on October 28, 2003, three employees were attempting to unjam a stuck roll-up door. Employee #1 and his coworker worked for a sub-contractor on the site. They were standing on a wooden pallet that was on a forklift driven by the third worker, an employee of the host employer. Employee #1 slipped and fell between 8 and 10 feet to the concrete floor, striking his head. He was hospitalized with a concussion.
