On January 4, 2004, Employee #1 was cleaning up a blow molder machine, which was not locked out. Employee #1 reached behind the machine and actuated it. As a result, he crushed his arm. In addition, he sustained bruises, contusions, and abrasions. Employee #1 was hospitalized.
