Employee's Arm Is Fractured While Clearing Jam in Machine

Employee #1 was clearing a jam in a machine when the pneumatic packaging ram engaged. She suffered multiple fractures to her right arm and was hospitalized. Further investigation revealed that there were no energy control procedures in place to control the pneumatic energy.
