At approximately 2:30 p.m. on June 29, 2008, Employee #1 was running a food packaging machine on the "Cool Cuts Line" using machine Number 2 in the "Specialties" department. Employee #1 experienced a problem with the drive chain of the Tiromat machine, causing the plastic trays to jam. Employee #1 was attempting to unjam plastic package trays while the machine was running. There were no guards to prevent contacting the nip points of the cross cutting operation.
