Investigation Summary
At approximately 2:30 p.m. on June 29, 2008, Employee #1 was running a food packaging machine on the "Cool Cuts Line" using machine Number 2 in the "Specialties" department. Employee #1 experienced a problem with the drive chain of the Tiromat machine, causing the plastic trays to jam. Employee #1 was attempting to unjam plastic package trays while the machine was running. There were no guards to prevent contacting the nip points of the cross cutting operation. The machine was not deenergized while it was being unjammed. Employee #1 sustained lacerations, fractures, and a partial amputation to fingers of her left hand. Employee #1 was released after 27 hours of hospitalization.
