Investigation Summary
At approximately 10:00 a.m. on July 11, 2008, Employee #1 and Employee #2 lifted the hood guarding the exhaust fan on the northeast corner of a roof of a single story facility. Employee #1 was attempting to check the belt tension on the belt and pulley for the electric motor running the exhaust fan. Employee #1 described the fan as "moving real slow", when he used his gloved right hand to check the belt tension. Employee #1's hand got caught in the nip point of the belt and pulley, causing an amputation and fracture of the distal phalanx on the fifth finger of his right hand. Employee #1 was transported to Irwindale Industrial Clinic. He was then transferred to Western Hand and Orthopedics, where he was presumably treated and released in less than 24 hours. The employer notified the Division in a timely manner.
