Employee Amputates Thumb While Cleaning Machine

On June 12, 2008, Employee #1 was working alone in the Slitter Rewinder Department cleaning the in-running rolls of the Slitter Rewinder Machine (Model Number 38MCS, Serial Number 3914823). The machine's function is to cut large rolls of poly film and cut them into smaller pieces. Some ink or wax was on the rolls. The rolls were approximately 60-in. long and were tension driven. Employee #1 attempted to clean the rolls with a rag while the machine was running. The in-running rolls were not guarded to prevent the entry of exposed employee's hands into the zone of danger, represented by a pinch point hazard. Employee #1's left thumb was pulled into the in-running rolls resulting in an amputation of the left thumb tip. Employee #1 was treated for his injury but was not hospitalized.
