Investigation Summary

On June 18, 2008, an employee was doing some repair work on a sorter machine in a Kaiser Refill Pharmacy in Downey, California. He was under the bed of a long table in production area troubleshooting a jam on the chain conveyor of a sorter machine, and his right index finger was pinched in a pinch point resulting in a partial amputation. The employee stated during interview that he was doing some trouble shooting work to clear a jam on the sorter machine in production area and his finger inadvertently contacted a pinch point of moving conveyor machine underneath the table. He was not a new employee and had performed this type of task several times in past. He also stated that the power to sorter machine could not be turned off as it was necessitated during troubleshooting process. He was the only service technician employee of the Knapp Logistics Automation Inc., working at Kaiser Facility without anyone's supervision when accident occurred. The employee was hospitalized and treated for partial finger amputation.
