Investigation Summary
At approximately 3:30 p.m. on November 5th, 2007, an employee and two coworkers were adjusting the alignment of packages in Automatic Blister Packaging Machine, Model Number DPP-250DII, Serial Number 1301. The employee's hand came into contact with the point of operation and the cutting dies came down on it. The employee suffered an amputated right palm and was transported to Los Angeles County USC hospital, where she was hospitalized and treated for her injury. The opening to the point of operation was 1.125 inches. There were several points of operation on the Automatic Blister Packaging Machine, none of which were guarded to prevent the operator's hands from entering them. There was no training and effective procedure for controlling the hazardous energy related to the Automatic Blister Packaging Machine.
