Investigation Summary

Investigation Nr: 202474300
Event: 07/25/2007
Employee Partially Blinded after Being Struck in Eye by Wood

At approximately 2:15 p.m. on July 25, 2007, Employee #1 was operating a table saw to make a cut on a wooden window frame. He used a piece of solid pine, 1.5-in. by 1-in. cross section by 28-in long, as a spacer between the shutter frame and the table saw's fence. After performing the cut, the spacer lumber, which was not secured to the fence, made contact with the running saw blade and kicked up. It struck Employee #1 in the left eye and caused partial blindness. Employee #1 was transported to Beverly Hospital, and later transferred to Kaiser Permanente Hospital where he was hospitalized for three nights. The cause of the accident was the failure to secure the wood spacer to the fence during the cutting operation. The cause of the serious injury was the failure to wear appropriate safety glasses or equivalent eye protection.
