Investigation Summary
At approximately 9:00 a.m. on May 25, 2007, a line operator found a jamming problem occurred at the carding section. He called an employee, a mechanic, to fix the problem. The employee and line operator turned off the machine, and locked the control panel, opened machine guards and started to trouble shoot the jamming problem. It was found that some of the carding wirings were bended in different directions and caused finished fabric running in different direction that caused jamming problem. The employee used one grinding blade to polish carding wiring on one cylinder. After the employee polished the carding wirings, he asked the line operator to turn on the machine on to check if the machine was functioning properly. The jamming problem still occurred. The employee told the line operator to turn off the machine and to lock the control panel. After the power of the machine was shut, the employee jumped on the platform on the top of the carding cylinders and started to polish carding wiring. Due to the momentum of these rollers, one of the rolls was still running slowly. It took them approximately 5-minutes to completely stop. While the employee was polishing carding wirings, somehow his right hand accidentally contacted the carding wiring of a carding cylinder. The carding cylinder grabbed his glove and pulled his right hand into the pinch point of rollers. The line operator and the plant supervisor ran to free the employee from the rollers of the carding machine. The employee's right hand was seriously injured with amputation of several fingers (not specified). The employee was hospitalized and treated for amputation of fingers.
