Employee Amputates Fingertip

At 10:30 a.m. on June 10, 2005, Employee #1 was cutting a 0.5-in. by 72-in.galvanized pipe, using a pipe cutting machine labeled D5. A guard that covers the gears inside the machine was not in place. As Employee #1 cut the pipe, he held it too close to the unguarded roller and his fingertip was cut off.
