Investigation Summary
At approximately 3:45 p.m. on October 11, 2006, Employee #1 and a coworker were processing laminate fiberglass sections. The fiberglass sections were cut into shape using a die press. The coworker was operating the die press, while Employee #1 extracted the cut fiberglass. The die press operated by the raising of the lower portion of the press rise to meet and compress the material against the die. The machinery used a two-hand control system to process and cycle the material. As the coworker was cutting the material, Employee #1 reached into the machinery to extract the material, when his right hand contacted the rotating parts of the machine. His right hand was crushed. He was taken to the hospital, where he received medical attention. Because of the severity of his hand injuries, medical personnel had to amputate four fingers. After the surgical procedures, he was hospitalized for postoperative care.
