Investigation Summary
At approximately 9:00 a.m. on September 19, 2006, Employee #1 was operating the Schaffer S 96-inch Rotary Grinder to grind hot roll steel for restaurant grill plates. Employee #1 changed the grinding segment under the supervision of a trainer co-worker. After the grinding segment had been changed, the trainer told Employee #1 to be sure to lower the dresser, and went back to his own work station. Employee #1 turned on the machine to rotate the grinding segment, and began to swing the dresser's arm. Employee #1 had forgotten to lower the dresser, and it contacted the rotation spindle and kicked back. His right hand was jammed between the dresser handle and the door frame adjacent to the dresser. He was transported by paramedic to the Beverly Hospital where he received surgery and was hospitalized for two days. He sustained serious fractures of the second through fifth fingers of his right hand.
