Investigation Summary

Investigation Nr: 202473021
Event: 07/27/2006
Employee Aputates Finger While Using Table Saw

At approximately 12:15 p.m. on July 27, 2006, Employee #1, a warehouse worker, was working for a kitchen countertop manufacturer. He was operating a Delta manufactured table-saw (Model Number 36-951) to cut a piece of lumber into .5-in. or .75-in. wide strips. After cutting the pieces, he tried to remove the strip piece from the saw blade, which he just cut. As he was holding the piece with his hand, the piece kicked backward toward the saw blade. This caused his hand to come in contact with the saw blade, which was operating at that time. His left middle finger was amputated. Employee #1 was treated at Western Hand Surgery Center without hospitalization. The guard on the table saw was attached to an arm (bar) instead of on the table of the saw, and the bar was secured to the side of the table-saw. The shop supervisor indicated that he observed immediately after the accident that the guard on the saw was placed on top of the guiding fence (a few inches above the table of the saw). It indicated the saw blade was exposed at the time of the accident.
