Employee's Arm Is Injured When Pinned by Mast of Forklift

At approximately 12:45 a.m. on September 19, 2004, Employee # 1 was performing preventive maintenance, changing a landing leg of a con-gear used for connecting a truck to a trailer. After finishing, he moved it to the yard using a Nissan Forklift, equipment Number ABF 93150. He raised the mask of the forklift and connected a hook to the connecting anchor of the con-gear. He exited the forklift to close the hook, tripped, and lost his balance. He fell into an opening between the mast and the fork. His leg hit the level control of the forklift, which lowered the mast down, pinning his right arm between the mast and the fork. Employee #1 sustained a tear of his right biceps.
