Investigation Summary
At approximately 5:00 p.m. on November 24, 2005, Employee #1 was working as a machine operator and baker for the Country Village retail bakery in Montebello, CA. Country Village is a bakery that makes hamburger and hotdog buns. Their equipment includes mixers, ovens, and machines for packaging the product. Employee #1 had been employed at Country Village for three years. At the time of the incident, he was removing jammed dough from the Model K machine, which made hamburger buns, while the machine was still running. The dough was jammed above the in-running rollers. As Employee #1 attempted to remove the dough, it dropped, and its weight pushed his right hand in between the rollers. His right hand was crushed. He was taken to the Beverly Hospital in Montebello, CA. He remained in the hospital for more than 24 hours, and underwent surgery for a crushing injury to his right hand. His supervisor had not been in the area at the time of the accident.
