Employee died after fall from scaffold

On November 14, 2003, an employee was installing 1-in. by 3-in. wooden strips overhead on which the ceiling drywall would be secured to. The wooden strips were being attached by employee using an air operated nail gun. Because the ceiling was 12 ft high, the employee was standing on a "baker scaffold" 5 ft 6 in. above concrete floor in a residential home under construction. The employee lost his balance while standing on the scaffold and started to fall backwards. He grabbed the wooden strips overhead but they broke and he fell backward onto the concrete floor striking his head. He was killed.
