Investigation Summary

Investigation Nr: 202453007
Event: 08/05/2006
Employee's Fingers Are Amputated by Pipe Grooving Machine

At approximately 9:10 a.m. on August 5, 2006, Employee #1 was an apprentice pipefitter working in the fabrication shop of the Murray Company. He was operating a Victaulic grooving machine. The machine used hydraulic pressure to create a groove on steel pipes by using a two-piece die set. He set-up the machine and was using an 8-in. diameter, 5-in. long piece of pipe as a test piece to set the groove depth. He had made two passes and had adjusted the machine for a third pass. He was holding the pipe in his gloved, right hand. As the pipe rotated, Employee #1 held the pipe. The glove became caught, and his fingers were pulled into the point of operation between the inner wall of the rotating pipe and the female die. He was hospitalized at Long Beach Memorial Emergency for treatment of the amputation of his index, middle and ring fingers. He was released later the same evening.
