Employee Is Injured When Struck by Falling Bundle of Paper

On May 19, 2006, Employee #1 was cleaning up an area in the warehouse. There was also a frontend wheel loader in the warehouse loading bundles of compacted paper into a freight container. There were five bundles of paper piled up when the frontend wheel loader somehow triggered the top two bundles to fall, striking Employee #1. It was determined the frontend loader was picking up the bundles and somehow a piece of wire became tangled with the bucket and triggered the top two bundles to fall as the bucket was been lifted. Each bundle of paper weighed approximately 1,000 lbs. Employee #1 was hospitalized, where he was treated for his injuries.
