Employee's Hand Is Injured in Conveyor

At 1:02 p.m. on March 11, 2006, Employee #1 was performing maintenance on a worm conveyor that was making a noise and not moving the material up the conveyor. He turned off the conveyor at the on/off switch that is located on the ladder support frame of the conveyor, approximately 8 ft away from the top of the auger. He climbed a ladder and removed the access cover of the auger at the top of the conveyor. He inserted his right hand into the housing of the worm conveyor to determine whether the bolts that secure the worm to the shaft of motor had sheared off. Employee #1 was injured, when his helper turned the conveyor on and the auger rotated, catching Employee #1's hand between the auger and the housing. He was hospitalized at Harbor UCLA Hospital for five days for a crushed right index finger, fractured right ulna, and torn ligaments in his right hand.
