Investigation Summary

Investigation Nr: 202452058
Event: 12/15/2005
Employee Injures Hand While Operating Grinding Machine

On December 15, 2005, Employee #1 was operating a Cincinnati manual centerless grinder to manufacture dowel pins. The grinder is designed to produce dowels that measure between 4-inches and 10-inches long. During the fabrication of these metal pieces, workers are required to manually feed each work piece into the point of operation with their hands. While Employee #1 was using the grinder to produce a dowel, measuring 1.25 inches in outside diameter by 6-inches long, his left index finger got caught between the grinding wheel and the dowel being produced. Employee #1 was transported to an orthopedic hospital in Los Angeles, where he was treated for his injury and released. The accident investigation revealed that the point of operation of the machine was not properly guarded for safe operation by employees.
