Investigation Summary
On November 28, 2005, Employee #1 was working for Douglas Steel Supply, a steel products manufacturer. There were a total of three Stamco slitters on the production floor identified as the small, the medium and the big by the operators, according to their capacities to process the coils. The employee was standing at the medium slitter's operator stand by the control panel, when he saw and determined that the slit width of the coils needed to be adjusted. He went over to the crank and attempted to manually adjust the slit width by hand cranking the spacers. While he was hand cranking the spacers, he somehow lost his balance and fell toward the running slitter heads. His left 3rd, 4th and 5th fingers were severed, when his left hand landed in between the rollers and made contact with the knives. His left thumb and index fingers were eventually amputated, because the doctors were unable to save them. Employee #1 was hospitalized. It was determined that the accident was caused by not safe guarding the slitter heads, where the knives meet the material coming in from the uncoiler by either restraining the operators' hands from inadvertently reaching into the point of operation or by inhibiting the slitters, whenever the operators' hands are drawn into the point of operation.
