Employee Amputates Fingers in Machinery

At approximately 9:45 a.m. on September 19, 2005, Employee #1, a machine operator, was operating the garlic bread wrapper for a manufacture that produced fresh and frozen bakery goods. The foil-type wrapping material had gotten jammed within the machine. After removing the jam, the foil wrapper was fed into the machine while it was still running. Employee #1 realigned the wrapping material and guiding the material towards the heat seal rollers. When he pushed the green start button, his right hand became entangled within the wrapping material and pulled into the point of operation. His right thumb, right index finger and right middle finger contacted the combination of heat seal and cutting rollers, resulting in their amputations. Emergency medical personnel were summoned. They arrived and transported him to a hospital for medical attention. At the medical center, he underwent surgical procedures for the amputating injuries, and was hospitalized for postoperative care and then released for home medical treatment. The investigation revealed that the employer did not establish written procedures for locking out the garlic bread machine during set up operations, and the employee did not properly lock out the garlic bread wrapping machine prior to set up.
