Event: 08/22/2005
Employee Is Struck by Falling Pallet and Injured

At approximately 9:40 a.m. on August 22, 2005, Employee #1 was unloading skid pallets which were 2-ft by 3-ft by 5-ft high, and weighed approximately 1000 pounds to 1600 pounds each. Employee #1 was moving the skid pallets using a manual pallet jack in place inside of a 26-ft Ford truck. A coworker was the operator of an industrial truck and was using it to move the skid pallets from the truck to the inside of the warehouse. The coworker picked up two skid pallets with the industrial truck and was backing out. The industrial truck went over a slight elevation and one of the skid pallets hit the dock plate and fell on Employee #1.
