Investigation Summary

Investigation Nr: 201059938
Event: 07/25/2005
Employee Is Injured While Engaged in Materials Handling

On July 25, 2005, Employee #1 and a coworker were lowering a metal box from its upright position. The box weight approximately 300 lbs and contained automobile parts. While Employee #1 and the coworker lowered the metal box down and were carrying its ends, the automobile parts shifted inside the box and rolled toward Employee #1's side. Most of the weight of the box relocated to Employee #1's side. Employee #1 yelled to the coworker that he needed to put the box down. As Employee #1 and the coworker began to lower the box to the floor Employee #1 did not move his left hand in time and his middle finger was crushed between the box and the floor. Employee #1 was taken to Memorial hospital in Gardena for evaluation and was later transferred to Western Hand Center, where he was hospitalized and underwent surgery to repair his left middle finger.
